Modify the login page

Hi,

I am planning to create a separate label in the login page of Openstack horizon. I found the changes need to be done in the /opt/stack/horizon/horizon/templates/auth/ but could someone please tell me how to bring two forms in a single page ? I need to bring a seperate label for federated login. I have attached my design please help
